Notice of Privacy Practices

This Notice of Privacy Practices ("Notice") describes how medical information about you may be used and disclosed, and how you can access that information. It is required by the Health Insurance Portability and Accountability Act (HIPAA) and is distinct from the website Privacy Policy. A final version will be provided and acknowledged during your first clinical encounter.

If you believe your privacy rights have been violated, you may file a complaint with the practice or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
